Failure Modes and Effects Analysis (FMEA) is a process for assessing the sequence of activities associated with a new process prior to full implementation. Through this process, potential process failures are identified by a cross-functional team (consisting of those involved with the process) and prevented prior to rollout; increasing the likelihood of successful implementation. FMEA involves walking-through a designed and flow-charted process prior to implementation. This enables all participants to understand the process from a cross-functional perspective and prevent potential issues (prioritized by likelihood and severity of occurence), avoiding rework, damaged customer relationships, staff frustration, and wasted resources. Once a process is finalized and successfully implemented, a documented record of the process is then kept as a future training reference or process improvement tool.
