Sample Community Health Center Monthly Reports



Below are links to sample Community Health Center Medical reports. Some reports are optional and provided based on the unique needs of each health center. Reports, which include score targets for key questions, are ideal for performance management and scorecards.

Sample Questionnaire: Developed in collaboration with community health center representatives, all questionnaires are customized around center needs and designed to be easily understood, is translated to Spanish, and incorporate HRSA recommended questions. Newer versions incorporate special "access to care" emphasis.

Administrative Aggregate Report: Combines all clinical area scores into an at-a-glace scorecard summary. Includes sample sizes, by-question response distribution, mean score rankings, standard deviations, loyalty scores, and detailed comments sorted by-clinic.

By-Specialty or By-Location Report: Provided for each clinical area/specialty to assess scores and trends. Used as a scorecard measure and improvement tool.

Cross-tabulation Analysis Report: A powerful analysis report to drill down scores and isolate patterns. Reports identify score variation by-specialty/location, gender, ethnic background, age, appointment classification, or other user-defined variables.

Subgroup Trend Report: Would you like to assess the impact of more focused improvement efforts, (such as cultural sensitivity towards a particular racial/ethnic group)? This custom report allows you to isolate improvement trends by-population subgroup.

Provider Scores and Comments Report: Identifies patient scores and comments by-Provider. A powerful tool for improving Provider-patient interactions. Can be presented with names or with coded physician identifiers for confidential group presentation.

Provider Score Comparison (Quarterly): Provides scores and statistics by-provider for key driver questions such as Exam Room Wait Time, Listening to Patient, and Explanation of Care. Can be provide with or without provider codes for confidentiality.

New Patient Report (NEW): Assess new patient first impressions. Report includes a comparison of new vs. established patient scores, new patient loyalty scores, new patient trends, and new patient scores by-location.

Access to Care Report (NEW): Assess scores for critical appointment scheduling and availability as well as wait time aspects. Assess trends and compare walk-in patient perceptions relative to appointment patients.

Reception/Front Desk Report (NEW): Assess scores for critical appointment scheduling and availability as well as wait time aspects. Assess trends and compare walk-in patient perceptions relative to appointment patients. Assess specific reception desks by-location or even by-desk (within a center).
